REVISED CALIFORNIA APPLICATION (LIA-CA) INCLUDES NEW STATE FORM.

Please replace LIA-CA with the revised version now packaged with LU1318 California-Option to Designate Additional Addressee to receive premium due notices on page 18. This form is not required but must be presented to the client.  Please begin using the new package immediately.

REVISED WILLIAM PENN APPLICATION (LIA-WP) INCLUDES REQUIRED ACCELERATED DEATH BENEFIT NOTICE.

Please replace LIA-WP with the revised version now packaged with ADB NOTE – Notice Regarding Acceleration of Death Benefit Payments on page 17. Both LIA-WP-NOTE- Notice to Proposed Insured (pages 2-3) and ADB NOTE (page 17) should be left with the applicant at the time of solicitation.  Please begin using the new package immediately.

APPLICATIONS AND PART 2 MEDICAL EXAM FORMS NOW INCLUDE LGA IDENTITY. 

Our ongoing effort to rebrand our forms with the Legal & General America logo continues — Disclosure forms, Privacy and HIPAA policy forms, Underwriting Questionnaires and other miscellaneous forms were completed last month. All state variations of our Banner and William Penn application and medical exam forms have now also been updated. Find them all on our website at LGAmerica.com.  There were no changes to the form numbers and descriptions. We will accept the former versions of these forms until October 28, 2013.
